Philadelphia County

Beyond Basic Care provides skilled nursing, home health aide services, and non medical home care throughout Philadelphia County. We work with patients transitioning from hospitals, rehabilitation facilities, and physician offices to ensure safe continuity of care at home. Services are coordinated in accordance with physician orders and individualized plans of care.

Delaware County

In Delaware County, we support individuals requiring skilled home health services as well as waiver based and private home care. Our team assists with Medicare eligible home health services and Community Health Choices coordination to help patients remain safely in their homes.

Chester County

Beyond Basic Care serves patients across Chester County with physician ordered skilled nursing, home health aide support, and daily living assistance services. We collaborate with healthcare providers to promote stable transitions and reduce unnecessary hospitalizations.

Bucks County

Residents of Bucks County can access both home health and supportive care services through Beyond Basic Care. Our services include skilled clinical monitoring, medication management, and personal assistance tailored to individual care plans.

Montgomery County

In Montgomery County, we provide coordinated home health and non medical support services for eligible individuals. Our team works closely with physicians, families, and care coordinators to ensure services are delivered safely and consistently in the home environment.
